Injury Participation - Disclaimer

I confirm that I have informed Elevations Academy of my child's injury, including any broken bone, fracture, sprain, or other condition that may affect their participation. I understand that my child is attending classes at my own risk and that I have sought appropriate medical advice regarding their ability to participate. I accept full responsibility for ensuring my child is fit to attend and will provide any relevant medical guidance or restrictions to Elevations Academy. I understand that Elevations Academy will make reasonable adjustments where possible; however, staff cannot provide medical supervision or treatment. I acknowledge that participation in dance involves physical activity and carries an inherent risk of further injury. By choosing for my child to attend, I accept these risks and agree that Elevations Academy, its teachers, employees, and volunteers shall not be held liable for any worsening of the existing injury or any related complications, except where liability
